The U.S. medical-care system is based principally on the private practice of medicine and job-related health insurance. In the early 1990s, there were about 9 million people employed in the system, of whom 600,000 were physicians (including 35,000 doctor of osteopathy), over 2 mln. nurses, and nearly 150,000 dentists. Community hospitals were declining in number as academic medical centres enlarged and more ambulatory surgery was performed. In 1991 there were only 5,400 community hospitals, compared with 6,500 ten years earlier, and only 900,000 instead of 1 mln. hospital beds. In addition, there were nearly 500 long-term hospitals, with 200,000 beds, and 20,000 nursing homes, with 2 mln. beds.
The distinguishing feature of the U.S. system is its entrepreneurial nature. Physicians tend to be Private practitioners (although 30% of practicing physicians are now full-or part-time members of health maintenance organizations, or HMOs). Pharmacies are profit-making, often independent shops. Manufacturers of pharmaceuticals, medical equipment, supplies and appliances are all in the private profit-making sector. "Voluntary" hospitals, on the other hand (such as community-run hospitals providing short-term care) are non-profit. About 15% of the total number of hospital beds is in "proprietary" hospitals (run for profit by investor-owned corporations). Long-term hospitals are often government units. Most psychiatric hospitals are run by the states. The Federal government operates all veterans' hospitals. To pay medical bills, the majority of the population relies on private insurance, which on average covers less than 40% of the costs. More than 30 mln. people over 65, and 3 mln. others under age 65 who are considered totally disabled, are covered in the government-financed program "Medicare", some part of whose premiums they must pay. Another 25 mln. receive medical care paid for by "Medicaid" the government-financed program for the "medically indigent". About 13% of the population, or 35 mln. people, are left without any coverage; they cannot purchase private insurance, and are not eligible for government programs.
The government does not guarantee receipt of care, even to those eligible for medical services. Everyone is expected to make his or her own arrangements when sick.
